What is a CMRA and why do I need to complete Form 1583? CMRA stands for Commercial Mail Receiving Agency. Before you can activate a Virtual Office or Business Address with Coconut Point Executive Center (CPEC), USPS regulations require you to complete, sign, and notarize Form 1583, and provide 2 forms of ID.
When you sign this form, you are authorizing CPEC to legally receive mail on your behalf. This protects your identity and helps prevent fraud.

How to Complete USPS Form 1583 – Follow the instructions below carefully:
General Notes:
• Use BLOCK LETTERING
• Submit TWO forms of ID (details below)
• You must sign the form in the presence of a notary public, (can often be found at banks or UPS Stores).
*Required Sections to Complete:
- Section 4: Applicant Information
- Enter your full name, home address, phone number, and email.
- The address listed must match one of your ID documents.
- Section 5 & Section 10: Authorized Individual (Optional)
- Fill out only if someone other than you will pick up your mail.
- Section 6: Mail Forwarding (Optional)
- Section 7: Business/Organization Information (Required for business use)
- 7c: Business Street Address
- Only fill this out if you have a separate business address that is not the same as the CMRA (CPEC) address.
- Leave this section blank if your business uses the CMRA as its main address or if you operate from home.
- 7c: Business Street Address
- Section 8–11: Identification
- Provide two valid IDs:
- Photo ID (e.g., driver’s license, passport, military ID)
- Address verification (e.g., lease, utility bill, insurance, vehicle registration)
- Driver’s license may only be used for one purpose: either photo ID or address verification, not both.
- Do not submit: Social Security cards, credit cards, or birth certificates.
- Provide two valid IDs:
- Boxes 12, 13 & 14: Fill in if applicable otherwise leave blank (for each name given we need 2 forms of ID).
- Box 15: Notary’s signature and seal (must be on this form).
- Box 16: Your signature. (must be signed in front of notary)
Once the form has been completed and notarized, send the original copy (NOT a photocopy) of the form along with photocopies of your ID documents to the CPEC mailing address.
Alternatively, you can email a scanned version of the original form to us with copies of your ID documents. Please ensure all text and identification is legible. We will submit the form and identification as required by law, and keep a copy for our records.
How do I find a notary to authorize CMRA Form 1583?
Your bank is always a good option.
Common Mistakes to Avoid

❌ Listing your Virtual Office address as your business’s place of registration
